Encore Preferred Provider Organization (PPO)

• Encore Health Network is one of the largest Preferred Provider Organizations (PPOs) in Indiana. There are currently more than 609,000 members in the network.
• Plan design requirements include a 20% benefit differential between in and out of network. Dual Choice offerings may be made with other networks.
• Currently, we have more than 170 participating hospitals and more than 31,000 participating physicians in Encore Health Network to choose from.

Encircle

• Encircle began in central Indiana and is growing statewide. The benefits of Encircle include claims savings, deeper provider discounts, reinsurance discounts, and flexibility.
• Plan design requirements include a 30% benefit differential between in and out of network. When a dual choice with Encore is offered, a minimum of $50.00 per month premium differential is required to encourage Encircle use.
• Currently, we have over 40 hospitals and 13,000 physicians in the Encircle network.

Encircle/Encore Combined

• Encircle/Encore Combined gives employees access to the entire Encore network of hospitals and providers while empowering them to obtain greater savings when they choose Encircle hospitals.
• Plan design requirements include a 20% Encore hospital differential or a 30% Encircle hospital benefit differential between in and out of network. In addition, Encore hospitals will also require a $600 deductible.
• Offers incentives for employees to use hospitals with lower fixed cost DRG pricing.
