Provider Update Form

Encore strives to maintain current provider information to ensure the accuracy of our provider directories. If you add a physician, change provider information or decide to term with Encore, fill out the appropriate form listed below and fax to 317-963-9850 or email the form to your Encore network specialist. If you are unsure of your network specialist, a list of Encore staff members are listed beneath Provider Update Forms.

You are about to download an Excel file. If you wish to email provider updates to an Encore network Specialist, please follow these four easy steps.

1. Open your email account. Select the appropriate Encore specialist from the list below. Copy and paste email address into appropriate field.
2. Download the Excel file form by clicking the appropriate link below.
3. Fill out the form completely and save it to your desktop.
4. Attach the Excel file to the email and send to Encore.
